Azilsartan falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the gist: Azilsartan (Edarbi) already lowers your blood pressure by relaxing your blood vessels. Danshen, in animal studies, seems to do something similar and can nudge blood pressure down too. Stack the two together and the worry is that your pressure could drop a little lower than intended.

If that happens, you might feel lightheaded, dizzy, tired, or unsteady, especially when you stand up quickly. I want to be honest about the evidence though: this is based mostly on lab and animal research, not solid human studies, so it's more of a theoretical caution than a proven problem. Azilsartan is not a prodrug, so this isn't about how it's broken down. Just loop in your pharmacist or doctor before adding Danshen, and watch how you feel.

Azilsartan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist with Danshen and Edarbi (azilsartan). Azilsartan is partly cleared by a liver enzyme called CYP2C9. In lab studies, compounds in Danshen seem to slow that enzyme down. If that held true in people, your body might clear azilsartan a little slower, letting more of it build up. Since azilsartan lowers blood pressure, the main concern would be it working too well, things like dizziness, lightheadedness when you stand, unusual tiredness, or blood pressure dropping lower than you want.

Now the reassuring part: this is only seen in test-tube and animal research, and it has never been reported in actual patients. So it's more of a theoretical caution than a proven problem. Still, before adding Danshen, please run it by your pharmacist or doctor so they can keep an eye on your numbers.

What Is Azilsartan Used For?

Brand name: Edarbi

Azilsartan medoxomil tablets — sold as both the generic Azilsartan Medoxomil and the brand Edarbi — are approved to treat high blood pressure (hypertension) in adults. Bringing blood pressure down lowers the risk of serious cardiovascular events, especially stroke and heart attack.

The medication can be used on its own or alongside other blood pressure medicines. It is not a cure for high blood pressure, and managing the condition fully often involves lifestyle steps like limiting sodium, exercising, and quitting smoking.
