Why this applies to Ramelteon

Ramelteon falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for Rozerem (ramelteon): your liver clears it using an enzyme called CYP1A2. Danshen has been shown in lab and test-tube studies to slow that same enzyme down. If that slowdown happened in your body, ramelteon could hang around longer and hit harder, meaning more next-day grogginess, dizziness, or feeling foggy in the morning. Ramelteon is not a prodrug, so blocking the enzyme would mean more drug effect, not less.

The reassuring part: this is only theoretical right now. When researchers tested danshen against another CYP1A2 drug in real people, they didn't see a meaningful effect. Still, since both can affect how you feel, it's worth a quick chat with your pharmacist or doctor before combining them.

Here's the short version: your body uses a liver enzyme called CYP2C9 to help clear ramelteon (Rozerem) out of your system. Lab studies suggest that compounds in danshen can slow that enzyme down. If that happened in real life, ramelteon could build up a bit and hit harder than expected, meaning more next-morning grogginess, dizziness, or feeling unusually drowsy or foggy. Ramelteon is not a prodrug, so this points toward a stronger effect, not a weaker one.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing about Rozerem (ramelteon): your body relies on a liver and gut enzyme called CYP3A4 to break it down. Danshen seems to mess with that enzyme in two different ways depending on how long you've been taking it. In one small study, taking danshen for 10 to 14 days actually sped up this enzyme, which would likely clear ramelteon out faster and could leave you tossing and turning, since the sleep effect may be weaker than expected.

Oddly, a single dose did the opposite and raised levels, which could mean more next-morning grogginess or dizziness. Because the direction can flip, it's hard to predict for any one person. Please check with your pharmacist or doctor before pairing these, rather than changing anything on your own.
