Rifabutin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: your body clears rifabutin partly through a liver and gut enzyme called CYP3A4. In one small study of healthy men, taking Danshen for 10 to 14 days actually sped up this enzyme, which lowered blood levels of a test drug by a lot. If the same thing happens with rifabutin, your levels could drop, and a weaker antibiotic may not control the MAC infection or help clear H. pylori as well.

One twist: a single dose of Danshen did the opposite and pushed levels up, which could mean more side effects like nausea, an upset stomach, or harmless orange-tinged urine. The evidence here is limited, so don't panic, but please loop in your pharmacist or doctor before combining them, especially since steady antibiotic levels really matter.

What Is Rifabutin Used For?

Brand name: Mycobutin

Rifabutin capsules are used to prevent disseminated MAC disease — a serious bacterial infection caused by Mycobacterium avium complex — in patients with advanced HIV infection.

It is not approved to treat active tuberculosis, and it should not be used alone against active TB. It is also not proven to prevent M. tuberculosis infections.
