Why this applies to Vinblastine

Vinblastine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing to know if you're considering Danshen alongside Vinblastine: this chemo drug is cleared from your body by a liver and gut enzyme called CYP3A4, and Danshen can mess with that enzyme. Vinblastine is active on its own (not a prodrug), so the worry is mostly about how much sticks around.

Taken regularly for a couple of weeks, Danshen tends to speed up clearance, which could lower vinblastine levels and possibly weaken your cancer treatment. But a single dose can do the opposite and push levels up, raising the risk of side effects like low blood counts, infections, nerve tingling, or constipation. The evidence is limited and comes mostly from a midazolam study, but with chemo even small swings matter. Please don't add Danshen without first clearing it with your oncologist or pharmacist.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Vinblastine. Your body normally uses a kind of cellular pump called p-glycoprotein to move this chemo drug out of cells and clear it. Danshen appears to slow that pump down. In a small human study it raised levels of another pump-dependent drug by roughly a third, so the worry is that Danshen could let Vinblastine build up higher than expected.

With a powerful chemo like this, even a modest bump in levels can matter. More drug hanging around can mean worse low blood counts, nerve tingling or numbness, constipation, and mouth sores. The evidence here is only moderate, but Vinblastine is too serious to gamble with. Please don't add Danshen without first clearing it with your oncologist or pharmacist.

What Is Vinblastine Used For?

Brand name: Velban

Vinblastine Sulfate Injection is used in the treatment of a range of cancers. Conditions that tend to respond well include generalized Hodgkin's disease, lymphocytic lymphoma, histiocytic lymphoma, advanced mycosis fungoides (a type of skin lymphoma), advanced testicular cancer, Kaposi's sarcoma, and Letterer-Siwe disease (a rare immune condition also called histiocytosis X).

It may also be used for cancers that respond less reliably, such as choriocarcinoma resistant to other chemotherapy drugs, and breast cancer that has not responded to hormonal therapy. Vinblastine is often given as part of a combination chemotherapy regimen alongside other cancer medicines to improve response rates.
