Why this applies to Cabotegravir

Cabotegravir falls under GLUCURONIDATED DRUGS — the group Danshen interacts with.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the gist: cabotegravir (Apretude) is cleared from your body mainly through a process called glucuronidation, handled by an enzyme family known as UGT (mostly UGT1A1). Danshen has been shown in lab dishes to nudge these enzymes in both directions, sometimes speeding them up and sometimes slowing them down, depending on how the root was processed. Because of that mixed picture, we can't reliably say which way it would push your cabotegravir levels.

The important reassurance: this is theoretical only, based on test-tube research, and has never actually been reported in people. Still, since cabotegravir is HIV medicine where steady drug levels matter for it to work, I'd run any Danshen use past your HIV doctor or pharmacist first rather than combining them on your own.

About Cabotegravir

What Is Cabotegravir Used For?

Brand name: Apretude

Vocabria (cabotegravir) tablets are used in combination with rilpivirine (Edurant) to treat HIV-1 infection in adults and adolescents aged 12 and older who weigh at least 35 kg and whose HIV is already virologically suppressed (virus levels undetectable or very low on their current regimen). Vocabria tablets serve as a short-term oral lead-in — taken for roughly one month — before switching to long-acting injectable therapy.

Cabotegravir tablets are also used as an oral lead-in for HIV-1 pre-exposure prophylaxis (PrEP) — helping HIV-negative adults and adolescents (weighing at least 35 kg) confirm they tolerate the drug before starting injectable PrEP. Both uses require confirmed HIV-negative status before starting.
