Why this applies to Tadalafil

Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: your body relies on a liver and gut enzyme called CYP3A4 to clear tadalafil out of your system. Danshen seems to nudge this enzyme in two different directions depending on how you take it. With steady use over a couple of weeks, danshen appears to speed up CYP3A4, which could lower tadalafil levels and make it work less well (a weaker or shorter response). But a single dose of danshen does the opposite and can raise levels, which may mean more side effects like headache, flushing, a stuffy nose, back pain, or lightheadedness from a bigger drop in blood pressure.

This is based on early human studies using midazolam, not tadalafil directly, so it's a possible interaction rather than a sure thing. Please run it by your pharmacist or doctor before combining them.

What Is Tadalafil Used For?

Brand names: Cialis, Tadliq

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.

Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
