Zanubrutinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Brukinsa (zanubrutinib). Your body uses a kind of cellular bouncer called p-glycoprotein to help move this medicine along and clear it. Danshen appears to slow that bouncer down. In one study, danshen pushed levels of another p-glycoprotein drug up by roughly 30 to 40 percent, so it's reasonable to think zanubrutinib levels could creep up too.

For a cancer drug like this, a bit more in your system can mean a bit more risk of side effects, things like bruising or bleeding, low blood counts, infections, or feeling unusually tired. Danshen on its own may also affect clotting, which adds to that bleeding concern. This is moderate, possible, not certain. Please don't start or stop anything on your own. Loop in your oncology pharmacist or doctor first.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with these two together. Your body clears zanubrutinib (Brukinsa) using a liver and gut enzyme called CYP3A4, and Danshen can mess with that enzyme. The tricky part is that Danshen can pull in two directions. When taken regularly for a week or two, it seems to speed that enzyme up, which could clear zanubrutinib faster and possibly weaken how well it controls your lymphoma or leukemia. A single dose may do the opposite and let levels climb, which could mean more bruising, bleeding, infections, or low blood counts.

This is moderate-strength, early human evidence, so it isn't fully nailed down. Since zanubrutinib is a serious cancer medicine, please don't add or stop Danshen on your own. Run it by your oncologist or pharmacist first.

What Is Zanubrutinib Used For?

Brand name: Brukinsa

Brukinsa (zanubrutinib) is used to treat adult patients with certain blood cancers. It treats mantle cell lymphoma (MCL) in patients who have had at least one prior therapy, Waldenström's macroglobulinemia (WM), relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 treatment, and chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Brukinsa is also used — in combination with another medicine called obinutuzumab — to treat relapsed or refractory follicular lymphoma (FL) in adults who have already received two or more lines of treatment. Several of these uses were granted accelerated FDA approval based on response rates, meaning confirmatory trials are still ongoing.
