Overview

Inositol nicotinate (also called inositol hexaniacinate or inositol hexanicotinate) is a man-made compound that joins inositol with several molecules of niacin (nicotinic acid, a B vitamin). It is not a plant or herb — it is a chemical combination made in a lab.

It is often sold as a "no-flush niacin" because, unlike regular niacin, it tends to cause less of the warm, red flushing feeling on the skin. The idea is that the body slowly breaks it down to release niacin and inositol over time.

People commonly take it as a supplement for cholesterol, circulation, and conditions involving poor blood flow.

How it works

Once swallowed, inositol nicotinate is thought to slowly break down in the body to release niacin (nicotinic acid) and inositol. Niacin can affect how the body handles fats and may widen small blood vessels, which is the basis for the cholesterol and circulation claims.

Because the niacin is released slowly, blood levels rise more gently — which is why it causes less flushing. However, this same slow release may also mean it delivers less of niacin's cholesterol-changing effect.

These mechanisms are based largely on what is known about niacin in general and on limited human research. Much remains uncertain.

Inositol nicotinate is generally tolerated in the short term, but it has not been thoroughly studied for long-term safety. Because it is a niacin compound, high doses could potentially affect the liver, blood sugar, and uric acid levels.

Orally, inositol nicotinate is converted to niacin and is therefore likely to cause similar side effects as niacin. Niacin can impair glucose tolerance in a dose-dependent manner. Dosages of niacin 3-5 grams daily appear to increase blood glucose in patients with or without diabetes, while dosages of 1.5 grams or less have minimal effects. Because inositol nicotinate is converted to niacin, it may also impair glucose tolerance.

Dyslipidemia Cardiovascular

Orally, inositol nicotinate is converted to niacin and is therefore likely to cause similar side effects as niacin. A prostaglandin-mediated flushing reaction is a common adverse effect of oral niacin. A large pooled analysis of clinical studies shows that up to 70% of people may experience flushing with oral niacin. Although flushing can occur with doses of niacin as low as 30 mg daily, it is more common with the larger doses of 500-2000 mg used for treatment of dyslipidemia. Inositol nicotinate is often promoted as a "no-flush" niacin; however, this benefit has not been demonstrated in clinical trials.

Orally, inositol nicotinate is converted to niacin and is therefore likely to cause similar side effects as niacin. Niacin is associated with elevated liver function tests and jaundice, especially with doses of 3 grams daily or more, and when doses are rapidly increased.
