Hydrochlorothiazide, Losartan Potassium falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Hyzaar combines two blood pressure lowering medicines: losartan, which relaxes blood vessels, and hydrochlorothiazide, a water pill that helps your body shed extra fluid and salt. Danshen, in animal studies, also seems to lower blood pressure on its own. So the worry here is simple addition: pairing them could push your blood pressure lower than intended.
If that happens, you might feel dizzy, lightheaded, tired, or woozy when you stand up quickly. I want to be honest though, this concern comes mostly from lab and animal research, not solid human studies, so it's more of a theoretical caution than a proven problem. Still, please don't start Danshen without checking with your pharmacist or doctor first, and let them know if you notice any of those symptoms.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing about Hyzaar: it combines hydrochlorothiazide (a water pill) with losartan, a blood pressure medicine. Losartan is actually a prodrug, meaning your body uses an enzyme (mostly CYP2C9, with some CYP3A4) to turn it into its more active form. So this CYP3A4 story is a bit of a wrinkle, not a slam dunk.
The research on Danshen here is mixed. Taken regularly for a week or two, it seemed to speed up CYP3A4, which could slightly lower losartan's active form and make your blood pressure control a little weaker. A single dose did the opposite. Honestly, this is theoretical for your specific combo and the evidence is thin. Just loop in your pharmacist or doctor before adding Danshen, since it can also affect blood clotting.
