Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist: part of your heartburn medicine, ranitidine, is moved around the body by a little transporter called p-glycoprotein. Danshen appears to slow that transporter down. In a small study, danshen raised blood levels of another p-glycoprotein drug by roughly a third, so the same thing could happen with ranitidine, meaning a bit more of it stays in your system than expected.

Ranitidine is generally well tolerated, so even a modest bump usually isn't dangerous. Still, you might notice more of its mild effects, like headache, dizziness, or constipation. The evidence here is limited and comes from one lower-quality study, so don't panic. Just check with your pharmacist or doctor before pairing danshen with this medicine, especially since danshen can also affect blood clotting.

What Is Ranitidine Used For?

Ranitidine is used to treat several acid-related stomach and esophageal conditions. These include active duodenal ulcers (ulcers in the small intestine) and benign gastric ulcers (stomach ulcers), GERD (gastroesophageal reflux disease, or chronic acid reflux), erosive esophagitis (irritation and damage to the esophagus from acid), and pathological hypersecretory conditions — rare disorders where the stomach produces too much acid, such as Zollinger-Ellison syndrome and systemic mastocytosis. It is also used as maintenance therapy to keep ulcers and esophagitis from coming back after healing.

Over-the-counter products Acid Reducer and Wal-Zan tablets are specifically labeled to relieve heartburn associated with acid indigestion and sour stomach, and to prevent heartburn brought on by certain foods or beverages.
