Why this applies to Vincristine

Vincristine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with vincristine. Your body relies on an enzyme called CYP3A4 to break this chemo drug down, and Danshen seems to nudge that enzyme in different directions depending on how it's used. When taken steadily for a week or two, Danshen appears to speed up CYP3A4, which could clear vincristine out faster and possibly make your treatment less effective. A single dose seemed to do the opposite, slowing clearance and letting levels climb, which could worsen side effects like nerve pain, tingling, numbness, or constipation.

Either way, this isn't something to guess about with a cancer medicine. The evidence is only preliminary and based mostly on a different drug, but vincristine is too important to risk. Please loop in your oncologist or pharmacist before using Danshen.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the short version: Vincristine relies on a cellular pump called P-glycoprotein to move out of your cells and clear from your body. Danshen may slow that pump down. In a study with a different P-glycoprotein drug (fexofenadine), danshen raised blood levels by roughly 30 to 40 percent. If the same thing happened with vincristine, more drug could linger in your system.

Why that matters: vincristine is already known for nerve-related side effects like tingling, numbness, weakness, constipation, and low blood counts. Higher levels could make those worse. This is a moderate, possible interaction based on lower-quality evidence, so it's not certain, but vincristine is too important and too touchy to gamble with. Please loop in your oncology team or pharmacist before mixing in any danshen.

What Is Vincristine Injection Used For?

Vincristine Sulfate Injection is approved to treat acute leukemia. It is also used as part of combination chemotherapy regimens for Hodgkin's disease, non-Hodgkin's malignant lymphomas, rhabdomyosarcoma (a soft-tissue cancer), neuroblastoma (a cancer of nerve tissue), and Wilms' tumor (a kidney cancer in children).

Because it works best alongside other cancer medicines, vincristine is almost always given as part of a multi-drug treatment plan rather than on its own.
