The best-supported concern involves diabetes medications, since clinical research shows Tribulus can lower blood sugar in adults with type 2 diabetes, which could add to the effect of their medication and raise the chance of low blood sugar. Lithium also deserves attention because Tribulus may act as a mild diuretic, which could reduce how the body clears lithium and raise its levels, and lithium has a narrow safety margin. Overview

Tribulus (Tribulus terrestris) is a low-growing plant that produces small yellow flowers and spiky, hard fruit. Because of its sharp burrs it is sometimes called caltrop, puncture vine, or bindii. It grows in many warm and tropical parts of the world, including Africa, Asia, southern Europe, and Australia.

The plant has a long history in traditional medicine systems, including Ayurveda and traditional Chinese medicine. Today the fruit, leaf, and root are dried and made into capsules, tablets, powders, and liquid extracts. It is one of the most widely sold ingredients in supplements marketed for sexual health and sports performance.

How it works

Tribulus contains a mix of plant compounds, including steroidal saponins such as protodioscin, as well as flavonoids and alkaloids. Protodioscin is the compound most often credited with the plant's possible effects.

Researchers have proposed that these saponins may influence hormones such as testosterone and DHEA, or that they may affect blood flow and nitric oxide pathways that play a role in sexual function. However, much of this is based on laboratory and animal studies, and the findings have not been clearly confirmed in well-designed human trials.

Safety & precautions

For most healthy adults, Tribulus appears to be reasonably well tolerated when used by mouth for a short time. Still, high-quality long-term safety data are limited, and supplement quality can vary widely between brands.

People with hormone-sensitive conditions, prostate problems, diabetes, or kidney or liver disease should be cautious and talk to a healthcare provider first, since Tribulus may affect hormones and other body systems. There have been rare reports of liver or kidney problems linked to products containing Tribulus, so stop use and seek care if you feel unwell.

Pregnancy and breastfeeding: Avoid Tribulus if you are pregnant, because it may affect hormones and may not be safe for a developing baby. Avoid it while breastfeeding as well, since there is not enough reliable information to consider it safe.

Side effects

When side effects occur, they are most often mild and may include stomach upset, cramping, nausea, or diarrhea. Some people report restlessness or trouble sleeping.

More serious problems are rare but have been reported, including cases of liver injury and kidney issues with certain Tribulus-containing products. Some animal studies have raised concerns about effects on the heart or sperm at high doses. Seek medical care if you develop yellowing of the skin or eyes, severe abdominal pain, dark urine, or other unexpected symptoms.

Serious Adverse Effects (Rare):

Orally: Cases of liver and kidney injury, seizures, and chronic painful erection with impaired sexual function have been reported. Pneumothorax and bronchial polyp after consuming the spine-covered tribulus fruit have been reported.

Orally, tribulus can cause abdominal pain, cramping, nausea, vomiting, diarrhea, and constipation. However, in one study, the rates of these gastrointestinal complaints were similar for patients taking tribulus and those receiving placebo.

In one case report, a patient drinking tribulus water 2 liters daily for two days presented with lower limb weakness, seizures, hepatitis, and acute kidney injury. The patient's condition improved after hemodialysis and discontinuation of tribulus water.

In one case report, a patient drinking tribulus water 2 liters daily for two days presented with lower limb weakness, seizures, hepatitis, and acute kidney injury. The patient's condition improved after hemodialysis and discontinuation of the tribulus water. In another case report, a healthy male taking one tribulus tablet (unknown dose) daily for a few months for bodybuilding purposes developed hyperbilirubinemia followed by acute kidney failure 2-3 weeks later. The patient was managed with intravenous fluids and a low-salt, low-protein diet.

The patient's condition improved after hemodialysis and discontinuation of tribulus water.
