Based on the quality of the available evidence, the overall risk of a clinically significant Sulforaphane drug interaction appears low for most people. Although the interaction list is long and every entry carries a moderate rating, all of it rests on lab and animal research, and none of these effects has been reported in humans.

Interactions deserving the most attention

The interactions closest to mattering involve medicines cleared by certain liver enzymes. Sulforaphane may slow the enzymes that process CYP3A4 substrates and CYP2E1 substrates, which could in theory raise the levels of those medicines. For CYP1A2 substrates, the evidence points both ways, since sulforaphane may block that enzyme while a related compound from the same vegetables may boost it. None of this has been confirmed in people, so no single medication stands out as a proven concern.

What the rest of the list means

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.

Overview

Sulforaphane is a natural sulfur-containing compound found in cruciferous vegetables such as broccoli, broccoli sprouts, cabbage, kale, cauliflower, and Brussels sprouts. Broccoli sprouts and broccoli seeds are especially rich sources.

In the plant, sulforaphane is not present in its active form. Instead, the plant holds a precursor called glucoraphanin, which is converted into sulforaphane by an enzyme called myrosinase when the plant is chopped, chewed, or otherwise damaged.

Today, sulforaphane is popular as a dietary supplement. Many products are made from broccoli seed or sprout extract and are standardized to glucoraphanin. People take it hoping to support antioxidant defenses, general health, and the body's natural detoxification systems.

How it works

Sulforaphane is best known for activating a cellular pathway called Nrf2. In lab studies, turning on this pathway increases the body's own antioxidant and protective enzymes, which may help cells handle stress and harmful molecules.

Researchers have also seen sulforaphane influence inflammation signals and certain detoxification enzymes in laboratory and animal models. Some studies suggest it may affect how cells grow and repair.

It is important to remember that much of this information comes from test-tube and animal research. What happens in a lab dish does not always translate into meaningful benefits in people, so these mechanisms should be seen as proposed explanations rather than proven effects.

Safety & precautions

Sulforaphane consumed through normal amounts of cruciferous vegetables is considered safe for most people and is part of a healthy diet. Concentrated supplements are generally well tolerated in short-term studies, but their long-term safety is less well understood.

People who should be cautious include those with a known sensitivity to cruciferous vegetables and anyone with a medical condition or taking medications, who should check with a healthcare provider first.

For pregnancy, there is not enough safety information on concentrated supplements, so it is best to avoid sulforaphane pills and stick to normal food amounts unless your doctor advises otherwise. For breastfeeding, supplement-level doses have not been well studied, so use caution and favor dietary amounts. As always, talk to your pharmacist or doctor before starting any new supplement.

Side effects

Sulforaphane is usually well tolerated, especially when obtained from food. When side effects occur, they are most often mild and digestive in nature, such as:

  • Gas or bloating
  • Stomach upset or mild nausea
  • Changes in bowel habits

Some people may not like the taste or smell of broccoli sprout products. Allergic reactions are uncommon but possible. If you notice signs of an allergic reaction such as rash, swelling, or trouble breathing, stop using it and seek medical care. Because high-dose supplements are not as thoroughly studied, report any unusual symptoms to your healthcare provider.

Orally, sulforaphane has been reported to cause headache in one clinical study. In another clinical trial, insomnia and irritability were reported. Sulforaphane has also been associated with two cases of seizure in males with autism. In one case, a patient with a recent history of seizure experienced a seizure after taking sulforaphane for 3 weeks. In another case, a patient with seizures which had been well-controlled with antiepileptic drugs for more than 1 year experienced a seizure 3 weeks after discontinuing sulforaphane. It is unclear if sulforaphane precipitated seizures in these patients.

Orally, sulforaphane has been reported to cause increased weight gain. In one clinical study in males 13-27 years of age with autism, patients taking sulforaphane gained an additional 4 pounds on average over 18 weeks when compared with those taking placebo.

Dyspepsia Gastrointestinal

Orally, sulforaphane and sulforaphane-rich extracts can cause bowel discomfort, stomach upset, heartburn, and vomiting. A specific stabilized form of sulforaphane (Cavamax W6) caused mild stomach upset in 60% of subjects in a small study. However, in some clinical studies, the gastrointestinal side effects reported with sulforaphane did not occur more frequently than with placebo.

Dosing

There is no official, standardized dose of sulforaphane. Supplements vary widely in how they are made and how much active compound they actually deliver. Many products are labeled by glucoraphanin content or by 'active sulforaphane,' and the amount of usable sulforaphane can differ depending on whether the enzyme myrosinase is present.

Because of this variability, the best approach is to follow the directions on the product label and not exceed the recommended amount. Eating broccoli, broccoli sprouts, and other cruciferous vegetables is a natural way to get sulforaphane without a supplement.
