None of the listed interactions are rated major, and all rest on limited or theoretical evidence rather than well-documented human harm.
The strongest concern is with anticoagulant and antiplatelet drugs: at everyday amounts reishi does not seem to affect platelets, but higher doses do, so people on blood thinners have the most reason for caution about bleeding. Blood pressure medications also stand out, since reishi has lowered blood pressure in some, though not all, patients, and an added drop could matter for someone whose pressure is closely managed. With diabetes medications, animal research suggested a blood sugar effect, but studies in people with type 2 diabetes have not shown lower fasting glucose, making this a weaker concern.

Overview
Reishi (Ganoderma lucidum) is a tough, woody mushroom with a shiny, reddish-brown surface. It grows on trees in warm, humid parts of Asia, though much of what is sold today is cultivated commercially.
Reishi has been used in traditional Chinese and other Asian medicine for thousands of years. Because the raw mushroom is bitter and hard to eat, it is usually taken as a capsule, powder, extract, or tea. People take it for a wide range of reasons, most commonly to support the immune system and general well-being.
How it works
Reishi contains compounds that researchers believe may be responsible for its effects, including polysaccharides (such as beta-glucans) and triterpenes (such as ganoderic acids).
In laboratory and animal studies, these compounds appear to interact with immune cells and show antioxidant and anti-inflammatory activity. Triterpenes have also been studied for possible effects on cholesterol and blood pressure.
It is important to remember that most of this research is from test-tube or animal studies. What happens in a lab does not always carry over to real benefits in people, so these mechanisms remain largely theoretical.

Safety & precautions
Reishi is generally considered well tolerated when taken by mouth for a few months. However, its long-term safety has not been carefully studied.
People with bleeding disorders or who are scheduled for surgery should be cautious, since reishi may affect blood clotting. Those with low blood pressure should also use care. If you have a serious health condition or take prescription medicines, talk to your pharmacist or doctor first.
Because there is not enough safety information, reishi should be avoided during pregnancy and breastfeeding. Choose products from reputable brands, as supplements are not tightly regulated and quality can vary.

Most people tolerate reishi well, but possible side effects include upset stomach, dizziness, dry mouth, nosebleeds, and skin rash or itching.
There have been rare reports of liver problems, especially with powdered reishi products. Stop using it and seek medical care if you notice yellowing of the skin or eyes, dark urine, severe belly pain, or unusual tiredness. If you have any allergic reaction, such as trouble breathing or swelling, stop and get help right away.

Allergic rhinitis (hay fever) Pulmonary/Respiratory
Respiratory allergy to reishi spores can occur. Sore throat and runny nose have also been reported.

Orally, reishi mushroom can cause dryness of the mouth, throat, or nasal cavity, nausea, stomach upset, and, more rarely, diarrhea.

Hepatitis Hepatic
One case of hepatotoxicity and one case of fatal fulminant hepatitis have been reported in patients who had used reishi mushroom powder for 1-2 months. There is a case report of a 61-year-old male with hypereosinophilia associated with hepatic nodules following the use of reishi mushroom powder for about 2 months. Symptoms resolved after discontinuation of the product. Although these side effects were thought to be associated with the use of reishi mushroom powder, it is unclear if other factors played a role. The patient had been taking tegafur, gimeracil, and oteracil potassium for about 4 months following anterior resection for rectal adenocarcinoma but discontinued these agents and initiated reishi mushroom due to liver injury.

Dosing
There is no official standardized dose for reishi. Doses vary a lot depending on whether you use the whole mushroom, a powder, or a concentrated extract.
