From the HelloPharmacist Editorial Team · Updated July 2026Based on the available evidence, the overall risk of a clinically significant Lithium drug interaction is meaningful for many people, so this supplement deserves more caution than most. Roughly half of the listed interactions are rated major, the rest are moderate, and none are minor.
The strongest human-supported concerns are with diuretics and serotonergic drugs such as SSRIs. Diuretics, especially thiazide and loop types, can slow the body's removal of lithium, which may raise lithium levels and side effects. Combining lithium with serotonergic medicines has been linked to serotonin syndrome, and lithium may also mask its warning signs. Common pain relievers called NSAIDs can likewise reduce how well the kidneys clear lithium.
Many of the other listed interactions are theoretical or come from experience with prescription-strength lithium, and it is often unclear whether the smaller amounts in lithium supplements would cause the same effects in people. Even so, lithium levels depend on how the kidneys handle it and on the body's sodium balance, which is why so many blood pressure and mood medicines appear on this list.

Diuretic Drugs
Theoretically, taking lithium supplements with loop diuretics might increase lithium levels and adverse effects.
Thiazide diuretics and loop diuretics might reduce lithium excretion, particularly in sodium-restricted patients. If lithium is clinically indicated and other treatment options are unavailable or inadequate in patients using diuretics, lithium treatment can be initiated with extreme caution. Serum lithium should be measured frequently and the doses used should be the lowest dose ordinarily tolerated. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.
Serotonergic Drugs
Theoretically, taking lithium supplements with serotonergic drugs might both mask and increase the risk of serotonin syndrome.
In a case report, a 67-year-old female with depression and bipolar disorder using lithium in combination with selective serotonin reuptake inhibitors (SSRIs) and other medications developed serotonin syndrome with symptoms of deep tendon hyperreflexia, muscle rigidity, tremor, and hyperthermia. However, agitation, one classical symptom of serotonin syndrome, was lacking. This was thought to be due to masking by lithium toxicity. Lithium can increase serotonin levels, thus, combining serotonergic drugs with lithium might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders. It is unclear if this interaction would occur with the smaller doses found in lithium supplements.
Ace Inhibitors (Aceis)
Theoretically, taking lithium supplements with ACEIs might increase levels and adverse effects of lithium.
Concomitant administration of ACEIs with lithium may increase lithium concentrations. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.
Anticonvulsants
Theoretically, taking lithium supplements with anticonvulsants might increase the risk of neurotoxicity.
Drugs such as carbamazepine and phenytoin seem to increase the risk of neurotoxicity. It is unclear if this interaction would occur with the smaller doses found in lithium supplements.
Antipsychotic Drugs
Theoretically, taking lithium supplements with antipsychotic drugs might increase the risk of encephalopathic syndrome.
Encephalopathic syndrome has been reported in multiple patients taking prescription lithium and antipsychotics concomitantly. Symptoms have included weakness and lethargy, fever, confusion, and extrapyramidal symptoms. In some patients, resulting brain damage was irreversible. Although there is no established causal relationship between these symptoms and the combination of lithium and antipsychotic medications, there is a theoretical relationship. It is unclear if this interaction would occur with the smaller doses found in lithium supplements.
Calcium Channel Blockers
Theoretically, taking lithium supplements with calcium channel blockers might reduce lithium levels and might also increase the risk of certain adverse effects.
Calcium channel blockers might reduce lithium concentrations. Monitor lithium levels with concurrent use. Calcium channel blockers might also increase the adverse neurological and gastrointestinal adverse effects of lithium. It is unclear if these interactions would occur with the smaller doses found in lithium supplements.
Methyldopa (Aldomet)
Theoretically, taking lithium supplements with methyldopa might increase the risk of lithium toxicity.
Concurrent use of methyldopa with lithium increases the risk of lithium toxicity. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.
Methylxanthines
Theoretically, taking lithium supplements with methylxanthines might decrease lithium levels.
Xanthines such as aminophylline, caffeine, and theophylline (Theo-Dur, Theo-24, others) might increase the clearance of lithium. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.
Nonsteroidal Anti-Inflammatory Drugs (Nsaids)
Theoretically, taking lithium supplements with NSAIDs might increase lithium levels and adverse effects.
NSAIDs can decrease the renal clearance of lithium and increase lithium levels. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.
Phenothiazines
Theoretically, taking lithium supplements with phenothiazines might decrease the levels and clinical effects of phenothiazines.
Concomitant use of lithium with phenothiazines might reduce lithium concentrations. Lithium might also reduce phenothiazine concentrations, making the pharmacokinetic effect unpredictable. It is unclear if these interactions would occur with the smaller doses found in lithium supplements.

Lithium: Uses, Safety & Side Effects
Lithium is a naturally occurring metal that, in prescription form (lithium carbonate or citrate), is an FDA-approved, well-proven treatment for bipolar disorder. Low-dose supplement forms like lithium orotate are sold for mood support, but there is very little reliable evidence that they work, and even small amounts of lithium can be risky for some people. Overview
Lithium is a soft, silvery-white metal and a natural chemical element (atomic number 3). It is found in tiny amounts in rocks, soil, drinking water, and many foods, so most people take in small amounts every day through their diet.
In medicine, lithium is best known as a prescription drug. Forms such as lithium carbonate and lithium citrate are FDA-approved to treat bipolar disorder and are among the oldest and most studied mood-stabilizing medicines.
Lithium also appears in dietary supplements, usually in much lower doses and in different forms like lithium orotate or lithium aspartate. These are marketed for general mood and mental wellness, but they are not the same as the carefully dosed and monitored prescription versions.
How it works
Scientists do not fully understand exactly how lithium works in the brain. It is thought to affect several brain chemical messengers (such as serotonin and dopamine) and to influence signaling pathways inside brain cells.
Lab and animal research suggests lithium may also protect nerve cells and affect enzymes and proteins involved in mood and brain cell health. Many of these ideas are based on laboratory or animal studies and may not directly apply to the low doses found in supplements.

Safety & precautions
Lithium has a narrow safety margin, which means the difference between a helpful amount and a harmful amount can be small. With prescription lithium, doctors regularly check blood levels along with kidney and thyroid function.
People with kidney problems, thyroid disease, heart conditions, or those who are dehydrated should be especially careful. Salt intake and fluid balance can change how much lithium stays in the body.
Pregnancy: Lithium crosses the placenta and has been linked to heart and other birth defects. Avoid lithium during pregnancy unless a doctor decides the benefits outweigh the risks and monitors you closely.
Breastfeeding: Lithium passes into breast milk and may build up in a nursing baby. Avoid it while breastfeeding unless directed and supervised by a doctor.
Even supplement forms like lithium orotate are not fully studied for safety, and high or repeated doses could potentially cause lithium to build up. Always check with a pharmacist or doctor first.
Side effects
Common side effects of lithium can include nausea, stomach upset, diarrhea, increased thirst, frequent urination, hand tremor, weight gain, and feeling tired or foggy.
Over the long term, lithium can affect the thyroid and kidneys, so monitoring is important with prescription use.
Lithium toxicity is a medical emergency. Warning signs include severe tremor, confusion, slurred speech, vomiting, severe diarrhea, unsteady walking, drowsiness, or seizures. If these occur, seek emergency care right away. Because supplement labels may not warn about this, it is important to take any lithium product seriously.

Orally: Edema, fatigue, fine tremor, gastrointestinal symptoms, lethargy, muscle weakness, polydipsia, polyuria, skin conditions, vertigo, and weight gain.
Acne Dermatologic
Orally, lithium can cause or exacerbate skin disorders such as hair loss, acne, psoriasis, and rash. A case of Stevens-Johnson syndrome is also reported in a patient with bipolar disorder who took lithium carbonate at an unknown dose for 17 days.

Bipolar disorder Psychiatric
Abrupt discontinuation of lithium resulting in a rapid reduction in serum lithium levels can precipitate recurrence of bipolar symptoms. Lithium should be tapered gradually over at least 14 days.
Bipolar disorder Other
Orally, chronic use of lithium has been reported to cause an irreversible reduction in taste and smell in a patient with bipolar disorder who took 400-1000 mg/day for 4 months. Chronic use of lithium 1200 mg/day has also been reported to cause dysphagia in a 17-year-old patient with bipolar disorder; however, the patient's serum lithium level was in the toxic range and the adverse effect resolved once the level normalized. It is unclear if these effects would occur with the smaller doses found in lithium supplements.

Bipolar disorder Cardiovascular
Orally, lithium has been reported to cause bradyarrhythmia. A case of symptomatic bradycardia due to sinoatrial node dysfunction is reported in a patient with bipolar disorder who took lithium orotate 20 mg daily for 5 years, despite a serum lithium level in the therapeutic range. Deep vein thrombosis is also reported in 2 patients with bipolar disorder who experienced toxic serum levels of lithium. It is unclear if these effects are a concern with the smaller doses found in lithium supplements.
Bipolar disorder Neurologic/CNS
Orally, lithium can cause vertigo, muscle weakness, lethargy, fatigue, and a dazed feeling. These adverse effects often improve with continued use. Fine tremor can occur and may persist with continued use. Chronic use of lithium can cause mild cognitive and memory impairment, particularly in the presence of dehydration or hyponatremia. These long-term neurological adverse effects of lithium are potentially due to accumulation in the central nervous system even when blood levels appear within the therapeutic range. Lithium-associated hyperparathyroidism-induced hypercalcemia has resulted in hallucinations, confusion, insomnia, and agitation. A case of delirium and transient difficulty with word finding is reported in a patient with bipolar disorder treated with lithium 250-500 mg twice daily and 9 sessions of electroconvulsive therapy. A case of mild encephalitis/encephalopathy with a reversible splenial lesion (MERS) is also reported in a patient with bipolar disorder who had a toxic lithium level of 1.94 mEq/L. It is unclear if these effects would occur with the smaller doses found in lithium supplements.

Diabetes Renal
Orally, lithium can cause polyuria, polydipsia, and edema. Chronic lithium use has been reported to cause central or nephrogenic diabetes insipidus, hypocalciuric hypercalcemia, and nephrotic syndrome. Long-term lithium use is estimated to increase the odds of chronic kidney disease (CKD) by at least 2-fold and may contribute to CKD progression. It is unclear if these effects would occur with the smaller doses found in lithium supplements.

Diabetes Endocrine
Orally, chronic use of lithium has been reported to cause various endocrine disorders. Case reports associate chronic lithium use with hypothyroidism, hyperthyroidism, goiter, hyperparathyroidism, and diabetes insipidus. In one case report, a 68-year-old male with schizophrenia developed severe hypothyroidism resulting in myxedema coma after taking oral lithium carbonate. He recovered after discontinuation of lithium and administration of levothyroxine. At least two other cases of lithium-associated myxedema coma have been reported. At least 4 cases of lithium-associated hyperparathyroidism have been reported in females aged 53-68 years that had taken lithium for 24 years or more. These patients presented with hypernatremia, hypercalcemia, elevated serum creatinine, thyroid or parathyroid abnormalities, and nephrogenic diabetes insipidus. It is unclear if these effects are a concern with the smaller doses found in lithium supplements.

Dosing
There is no established, well-tested dose for low-dose lithium supplements, and product strengths vary widely between brands. If you use a supplement, follow the label directions and do not take more than recommended.
Prescription lithium doses are individualized by a doctor and adjusted using blood tests; these should never be guessed at or self-managed.
Because lithium can be harmful at the wrong amount, talk to a pharmacist or doctor before starting any lithium product, especially if you have a health condition or take other medicines.
