Based on the available evidence, the overall risk of a clinically significant Andrographis drug interaction appears low for most people, since every known concern comes from animal or laboratory research rather than proven effects in humans. None of the several hundred listed interactions is rated major, and all carry a low likelihood grade because they rest on theoretical findings, not human studies.

Interactions deserving the most attention

Three groups deserve the closest look. Because andrographis showed antiplatelet effects in lab and animal work, we would watch it most carefully alongside anticoagulant or antiplatelet drugs, where even a small added bleeding effect matters. Animal research also hints it could lower blood pressure, which could add to blood pressure medications. Finally, lab studies suggest it may stimulate the immune system, which could work against immunosuppressants, medicines that transplant patients and others rely on staying steady.

What the rest of the list means

The long list of moderate interactions does not mean each combination is harmful. Most reflect these same three theoretical concerns applied across many medications. In the few drug-specific animal studies, such as those with celecoxib and glipizide, the changes seen were small and their real-world importance is unclear.

Overview

Andrographis (Andrographis paniculata) is a bitter-tasting plant native to South and Southeast Asia, including India, Sri Lanka, and China. It has been used for centuries in traditional systems such as Ayurveda and Traditional Chinese Medicine. Because some people use it for immune-related complaints, it is sometimes nicknamed "Indian echinacea."

The leaves and the above-ground parts of the plant are used to make supplements. Andrographis is commonly taken for colds, flu, sore throat, fever, and general immune support. It is sold as capsules, tablets, tinctures, teas, and standardized extracts.

How it works

Andrographis contains a group of compounds called andrographolides, and many supplements are standardized to a certain amount of andrographolide. These compounds are thought to be responsible for most of the plant's effects.

Laboratory and animal studies suggest that andrographolides may have anti-inflammatory and immune-modulating properties, and may affect how the body responds to viruses. Some research also points to possible antioxidant effects.

It is important to know that much of this information comes from test-tube and animal research. How well these effects apply to people taking the herb is still not fully understood.

Safety & precautions

For most healthy adults, Andrographis appears to be reasonably well tolerated when used short-term in typical amounts. Still, long-term safety has not been well studied, and supplement quality can vary between brands.

People with autoimmune conditions may want to be cautious, since Andrographis may affect immune activity. Those with liver problems, bleeding disorders, or low blood pressure should check with a healthcare provider first. Anyone scheduled for surgery should mention its use to their care team.

Pregnancy: Avoid Andrographis during pregnancy. Some animal and laboratory data raise concerns about possible effects on reproduction. Breastfeeding: There is not enough reliable safety information, so it is best to avoid it while breastfeeding. As always, talk with your pharmacist or doctor before starting any new supplement.

Side effects

Andrographis is bitter, and the most common side effects are mild, including upset stomach, nausea, diarrhea, loss of appetite, headache, fatigue, and a bitter or unpleasant taste.

Less commonly, some people may have allergic reactions, such as rash, hives, or itching. Stop using it and seek medical care if you have signs of a serious allergic reaction, such as swelling or trouble breathing. There have also been rare reports raising concern about effects on the liver, so stop use and contact a provider if you notice yellowing of the skin or eyes, dark urine, or persistent stomach pain.

Andrographis: Reported Adverse Effects

Orally, andrographis is generally well tolerated. Adverse effects are more likely when doses reach or exceed 5-10 mg/kg of andrographolide content and when treatment duration exceeds 14 days.

Most Common Adverse Effects:

Orally: Abdominal discomfort, altered taste, diarrhea, dizziness, fatigue, headache, nausea and vomiting, pruritus, rash, and urticaria.

Serious Adverse Effects (Rare):

Orally: Severe allergic reactions, including anaphylaxis.

Orally, andrographis has been reported to cause headache, fatigue, anorexia, somnolence, insomnia, lethargy, malaise, and drowsiness. Headache and fatigue occurred more often with high doses of the andrographolide constituent (5-10 mg/kg daily) in one clinical trial.

Dermatitis Dermatologic

Orally, andrographis has been frequently reported to cause maculopapular, erythematous rash, pruritus, and urticaria. Andrographis consumption has also been reported to cause angioedema, exfoliative dermatitis, skin exfoliation, Stevens-Johnson syndrome, acute generalized exanthematous pustulosis, bullous eruption, fixed eruption, stomatitis, allergic purpura, flushing, and swelling.

Parenterally, there have been reports of maculopapular rash, urticaria, pruritus, and flushing with the use of andrographolide derivative injections; about one-third of patients experienced skin or subcutaneous reactions.

Diarrhea Gastrointestinal

Orally, andrographis has been reported to cause nausea, vomiting, diarrhea, dyspepsia, flatulence, altered or metallic taste, and abdominal discomfort. Andrographis intake has also been reported to cause epigastric pain, ulcerative stomatitis, melena, dry mouth, and dry lips.

Parenterally, there have been reports of diarrhea, nausea, vomiting, and abdominal discomfort with the use of andrographolide derivative injections; over 40% of patients experienced gastrointestinal events.

Orally, there is one case report of hepatitis associated with andrographis use.

Hypotension Immunologic

Orally, andrographis has been reported to cause anaphylactic shock in 2 cases with determined causality, and 7 cases with probable causality. Anaphylactic shock developed in 5 minutes to one day after oral intake, and included symptoms such as hypotension, chest pain, urticaria, angioedema, wheezing, and tachycardia. Additionally, andrographis intake has been associated with cases of eosinophilia and fever. High doses of the andrographolide constituent (5-10 mg/kg daily) have been associated with two cases of lymphadenopathy and three cases of lymph node pain.

Parenterally, there have been 97 cases reporting severe or life-threatening anaphylaxis after andrographolide derivative injections, 3 of which resulted in death.

Dosing

There is no single official or standardized dose of Andrographis. Products differ in how they are made and in how much andrographolide they contain, which makes general dosing recommendations difficult.

Many cold-related products are standardized to a specific amount of andrographolide and are usually taken for a short period at the start of symptoms. The best approach is to follow the directions on the product label and not exceed the recommended amount.

Because the right dose depends on the product and your personal health, check with a pharmacist or doctor before starting, especially if you take other medications or have a health condition.
