None of the listed interactions are rated major, and nearly all rest on lab or animal findings rather than confirmed effects in people.
The best-documented concern involves medications the body clears through a specific liver enzyme that peppermint oil can slow. In a human study, a single dose of peppermint oil raised blood levels of felodipine, a blood pressure medication processed this way. Animal research also suggests peppermint oil could raise levels of cyclosporine, a transplant medication where even small level changes matter, though this has not been reported in people. These deserve the closest attention.
Peppermint's long list of interacting medications looks daunting, but most entries are theoretical, based on lab studies showing peppermint may affect enzymes the body uses to clear many drugs. In several cases the predicted effect has not appeared in humans; for example, peppermint tea did not meaningfully change how people processed caffeine. A long list here mostly reflects possibilities, not proven problems.

Cyclosporine (Neoral, Sandimmune)
Theoretically, peppermint oil might increase the levels and adverse effects of cyclosporine.
In animal research, peppermint oil inhibits cyclosporine metabolism and increases cyclosporine levels. Inhibition of cytochrome P450 3A4 (CYP3A4) may be partially responsible for this interaction. An interaction between peppermint oil and cyclosporine has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, peppermint might increase the levels of CYP2C19 substrates.
In vitro research shows that peppermint oil inhibits CYP2C19. So far, this interaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, peppermint might increase the levels of CYP2C9 substrates.
In vitro research shows that peppermint oil inhibits CYP2C9. So far, this interaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, peppermint might increase the levels of CYP3A4 substrates.
Clinical research in healthy volunteers shows that a single dose of peppermint oil 600 mg inhibits CYP3A4 enzymes and increases the AUC of felodipine, a CYP3A4 substrate. However, in vitro research suggests that peppermint oil only inhibits CYP3A4 at very high concentrations.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, peppermint might increase the levels of CYP1A2 substrates.
In vitro and animal research shows that peppermint oil and peppermint leaf inhibit CYP1A2. However, in clinical research, peppermint tea did not significantly affect the metabolism of caffeine, a CYP1A2 substrate. It is possible that the 6-day duration of treatment may have been too short to identify a difference.
Peppermint: Uses, Safety & Side Effects
Peppermint is a popular herb with the best evidence supporting enteric-coated peppermint oil for easing IBS symptoms. It is generally well tolerated for most adults, but it can cause heartburn and is not recommended in concentrated oil form for infants and young children. Overview
Peppermint is a fragrant perennial herb from the mint family (Lamiaceae). It is actually a natural cross between watermint and spearmint, and it is now grown around the world. It is native to Europe and Asia and is easily recognized by its cool, minty smell and spear-shaped leaves.
People use peppermint in many ways. The leaves are brewed into tea, used in cooking, and added to candies and personal care products. The essential oil, which is pressed or distilled from the plant, is used in capsules, topical products, and aromatherapy. In supplements aimed at digestion, peppermint oil is often placed in an enteric-coated capsule so it travels further down the gut before being released.
How it works
The main active ingredient in peppermint oil is menthol. Menthol can relax the smooth muscle of the gut wall, which may explain why peppermint oil can ease cramping and pain in IBS. This muscle-relaxing effect is also thought to help with gas and bloating.
Menthol creates a cooling sensation by acting on temperature-sensing nerve receptors in the skin and mucous membranes, which is why peppermint feels refreshing and may give a mild numbing or soothing feeling. Laboratory studies also suggest peppermint compounds may have antioxidant and germ-fighting properties.
It is important to remember that many of these mechanisms come from test-tube and animal research. These findings help explain how peppermint might work, but they do not prove it will treat a specific condition in people.

Likely Effective Irritable bowel syndrome (IBS)
Oral enteric-coated peppermint oil is conditionally recommended by the American College of Gastroenterology (ACG) for the relief of global IBS symptoms, such as abdominal pain.
Possibly Effective Barium enema-related colonic spasm
Rectal peppermint, as part of barium enema preparations, seems to reduce colonic spasms during examination. Oral peppermint also seems to be beneficial.

Safety & precautions
Peppermint tea and the amounts used in food are generally considered safe for most adults. Concentrated peppermint oil is stronger and should be used with more care. People with gastroesophageal reflux disease (GERD) or frequent heartburn may find that peppermint makes their symptoms worse, because it can relax the muscle between the stomach and esophagus.
People with hiatal hernia or known gallstones should talk to a healthcare provider before using peppermint supplements. Never put peppermint oil directly under the nose or on the face of infants and young children, as menthol can cause serious breathing problems.
For pregnancy, peppermint tea in normal food amounts is likely fine, but concentrated oil and supplements have not been well studied, so it is safest to avoid them unless your provider approves. For breastfeeding, information is limited, so avoid concentrated peppermint supplements and check with your doctor or pharmacist first.
Side effects
Peppermint is usually well tolerated. The most common side effect of peppermint oil capsules is heartburn or a burning feeling. Enteric-coated capsules are designed to reduce this. Some people may also notice burping with a minty taste, nausea, or a burning sensation when going to the bathroom.
Applied to the skin, peppermint oil can cause irritation, redness, or allergic rash in some people, especially if it is not diluted. Rarely, people may have an allergic reaction. Swallowing large amounts of concentrated peppermint oil can be dangerous, so always follow product directions and keep essential oils out of reach of children.

Orally, topically, or rectally, peppermint oil is generally well tolerated. Inhaled,
peppermint oil seems to be well tolerated. Intranasally, no adverse effects have been reported. However, a thorough evaluation of safety outcomes has not been conducted. Orally, peppermint leaf seems to be well tolerated.
Most Common Adverse Effects:
Orally: Abdominal pain, anal burning, belching, diarrhea, dry mouth, heartburn, nausea, and vomiting.
Topically: Burning, dermatitis, irritation, and redness.
Asthma Immunologic
One case of IgE-mediated anaphylaxis, characterized by sudden onset of lip and tongue swelling, tightness of throat, and shortness of breath, has been reported in a 69-year-old male who consumed peppermint candy. An allergic reaction after use of peppermint oil in combination with caraway oil has been reported in a patient with a history of bronchial asthma. It is not clear if this reaction occurred in response to the peppermint or caraway components.

Burns Dermatologic
Topically, peppermint oil can cause skin irritation, burning, erythema, and contact dermatitis. Also, a case of severe mucosal injury has been reported for a patient who misused an undiluted over the counter mouthwash that contained peppermint and arnica oil in 70% alcohol.
In large amounts, peppermint oil may cause chemical burns when used topically or orally. A case of multiple burns in the oral cavity and pharynx, along with edema of the lips, tongue, uvula, and soft palate, has been reported for a 49-year-old female who ingested 40 drops of pure peppermint oil. Following treatment with intravenous steroids and antibiotics, the patient's symptoms resolved over the course of 2 weeks. Also, a case of chemical burns on the skin and skin necrosis has been reported for a 35-year-old male who spilled undiluted peppermint oil on a previous skin graft. Oral peppermint oil has also been associated with burning mouth syndrome and chronic mouth ulceration in people with contact sensitivity to peppermint. Also, excessive consumption of mint candies containing peppermint oil has been linked to cases of stomatitis.

Cluster headache Neurologic/CNS
Diarrhea Gastrointestinal
Orally, peppermint oil can cause heartburn, nausea and vomiting, anal or perianal burning, abdominal pain, belching, dry mouth, diarrhea, and increased appetite. Enteric-coated capsules might help to reduce the incidence of heartburn. However, in one clinical study, a specific enteric-coated formulation of peppermint oil (Pepogest; Nature's Way) taken as 180 mg three times daily was associated with a higher rate of adverse effects when compared with placebo (48% versus 31%, respectively). Specifically, of the patients consuming this product, 11% experienced belching and 26% experienced heartburn, compared to 2% and 12%, respectively, in the placebo group. A meta-analysis of eight small clinical studies in patients with irritable bowel syndrome shows that taking enteric-coated formulations of peppermint oil increases the risk of gastroesophageal reflux symptoms by 67% when compared with a control group. Enteric-coated capsules can also cause anal burning in people with reduced bowel transit time.

Fatigue Hepatic
One case of hepatocellular liver injury has been reported following the oral use of peppermint. Symptoms included elevated liver enzymes, fatigue, jaundice, dark urine, and signs of hypersensitivity. Details on the dosage and type of peppermint consumed were unavailable.

Dosing
There is no single official dose for peppermint, and the right amount depends on the form and the reason you are using it. For IBS, products usually come as standardized enteric-coated peppermint oil capsules with directions on the label. Teas are made by steeping the dried leaves in hot water.
Because product strength varies widely, the best approach is to follow the directions on your specific product and not to exceed the recommended amount. Concentrated essential oil should never be swallowed unless it is in a product made for that purpose. Always check with your pharmacist or doctor before starting peppermint supplements, especially if you have a health condition or take other medicines.
