Monoamine Oxidase Inhibitors (Maois)
Concomitant use of MAOIs with yohimbe can result in additive effects.
Yohimbine, a constituent of yohimbe, has MAO inhibitory effects. At high doses, yohimbine is a non-selective inhibitor of MAO.
Antihypertensive Drugs
Theoretically, yohimbe might reduce the effects of antihypertensive drugs.
Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control.
Clonidine (Catapres)
Theoretically, yohimbe might precipitate clonidine withdrawal.
Chronic clonidine use can downregulate alpha-2 adrenoreceptors. Animal research and one human case report suggest that concomitant administration of yohimbine, an alpha-2 adrenoceptor antagonist, may precipitate clonidine withdrawal and lead to sympathomimetic toxicity, including hypertensive crisis.
Cytochrome P450 2D6 (Cyp2D6) Inhibitors
CYP2D6 inhibitors may increase the levels and adverse effects of yohimbine, a constituent of yohimbe.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP2D6 isoenzymes. Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine and reduces the clearance of yohimbine compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers..
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, yohimbe might increase the levels and adverse effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of yohimbe bark, inhibits CYP2D6 enzyme activity.
Cytochrome P450 3A4 (Cyp3A4) Inhibitors
Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.
Paroxetine (Paxil)
Paroxetine decreases the clearance of yohimbine and may increase its effects.
Paroxetine, a cytochrome P450 (CYP) 2D6 inhibitor, increases the maximum serum concentration of yohimbine by about 350% and reduces the clearance of yohimbine by about 80% compared to yohimbine alone in patients who are extensive CYP2D6 metabolizers. No significant changes in pharmacokinetic parameters of yohimbine were observed with coadministration of paroxetine in patients who are poor CYP2D6 metabolizers.
Phenothiazines
Theoretically, using yohimbine with phenothiazines might have additive effects.
Yohimbine, a constituent of yohimbe, has alpha-2 adrenergic antagonist effects. Theoretically, combining it with phenothiazines can cause additive alpha-2 adrenergic antagonism.
Stimulant Drugs
Theoretically, taking yohimbe with stimulant drugs can have additive effects.
Yohimbine, a constituent of yohimbe, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. Theoretically, taking yohimbe with stimulant drugs can have additive stimulant and hypertensive effects.
Tricyclic Antidepressants (Tcas)
Theoretically, taking yohimbe with TCAs can increase adverse effects.
A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor. The effects of yohimbe bark itself are unclear; oral yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.
Anticoagulant/Antiplatelet Drugs
Theoretically, combining yohimbe bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, yohimbe might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that yohimbe extract induces CYP1A2 enzymes.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Overview
Yohimbe (Pausinystalia johimbe) is an evergreen tree that grows in the rainforests of western and central Africa. The bark has been used in traditional African medicine for many years, often as an aphrodisiac and to boost sexual performance.
The bark contains a chemical called yohimbine. This compound is also available as a prescription medicine in some countries (yohimbine hydrochloride). Over-the-counter yohimbe supplements are sold widely for sexual health, weight loss, and energy. However, the amount of yohimbine in these products can vary a lot and is often not accurately listed on the label.
How it works
The main active compound in yohimbe is yohimbine. Yohimbine blocks certain receptors in the body called alpha-2 adrenergic receptors. By blocking these receptors, it may increase blood flow and nerve signals to certain tissues, which is the proposed way it could help with erections.
Yohimbine can also raise the release of norepinephrine, a chemical messenger that increases heart rate and blood pressure. This same action is why it is promoted for energy and fat burning, but it is also the reason it can cause unwanted heart and blood pressure effects. Much of the detailed understanding comes from laboratory and animal research.

Safety & precautions
Yohimbe should be used with caution. It can raise blood pressure and heart rate and has been linked to serious reactions, especially at higher doses. A major concern is that supplement labels often do not reflect the true amount of yohimbine inside, so it is hard to know how much you are actually taking.
People with heart disease, high blood pressure, kidney disease, anxiety or other mental health conditions, or a history of seizures should avoid yohimbe unless a doctor advises otherwise. Older adults may be more sensitive to its effects.
Yohimbe should be avoided during pregnancy because it may affect the uterus and is considered unsafe. It should also be avoided while breastfeeding, since there is not enough information to know it is safe. Always check with a pharmacist or doctor before starting it.
Side effects
Common side effects can include dizziness, headache, nausea, anxiety, irritability, sweating, fast or pounding heartbeat, and increased blood pressure.
Rare but serious effects have been reported, especially with high doses, including very high blood pressure, severe heart rhythm problems, heart attack, seizures, and in extreme cases life-threatening reactions. Stop using yohimbe and seek medical care if you have chest pain, a very fast or irregular heartbeat, trouble breathing, severe anxiety, or fainting.

Most Common Adverse Effects:
Orally: Yohimbine, a constituent of yohimbe, has been associated with anxiety, agitation, diaphoresis, diarrhea, flushing, headache, hypertension, increased urination, nausea, tachycardia, tremors, vertigo, and vomiting.
Serious Adverse Effects (Rare):
Orally: Yohimbine, a constituent of yohimbe, has been associated with atrial fibrillation, hypertensive crisis, myocardial infarction, and QT interval prolongation.
Anxiety Psychiatric
Orally, yohimbine, a constituent of yohimbe, may increase malaise, fatigue, insomnia, restlessness, agitation, and anxiety. In a clinical study of healthy subjects, administration of yohimbine increased impulsivity, with larger doses increasing impulsivity more than 50%.

Atrial fibrillation Cardiovascular
Orally, yohimbine, a constituent of yohimbe, has been associated with hypertension, especially at higher doses. A case of hypertensive crisis was reported in a 63-year-old male taking a yohimbine-containing herbal product once daily for one month. The patient was successfully managed with intravenous nitroprusside followed by clonidine. Tachycardia, fluid retention, palpitations, and chest discomfort have also been reported. Conduction abnormalities have also been reported. There have been some reports of myocardial infarction, atrial fibrillation, and QT interval prolongation. In theory, these effects may also occur with the use of yohimbe bark extract.

Cluster headache Neurologic/CNS
Orally, yohimbine, a constituent of yohimbe, has been associated with reports of general central nervous system (CNS) and autonomic excitation, tremulousness, head twitching, seizure threshold changes, enhanced brain norepinephrine release, decreased energy, dizziness, vertigo, and headache. Cold feet and chills have also been reported with yohimbine. Other adverse reactions include flushing and diaphoresis. Excessive doses of yohimbine can also cause paralysis. A case of acute neurotoxicity characterized by malaise, vomiting, loss of consciousness, and seizures has been reported for a 37-year-old bodybuilder who ingested a single dose of yohimbine 5 grams. Improvement was seen within 12 hours following treatment with furosemide, labetalol, clonidine, urapidil, and gastrointestinal decontamination.

Cough Pulmonary/Respiratory
Orally, yohimbine, a constituent of yohimbe, may cause bronchospasm, tachypnea, cough, and rhinorrhea. A case of sinusitis characterized by pain and discomfort above both eyes has been reported for a 59-year-old male taking yohimbine 5.4 mg three times daily to treat erectile dysfunction. Symptoms resolved within 24 hours of discontinuing yohimbine. The effect was attributed to the alpha-2 adrenergic antagonist effects of yohimbine. Excessive doses of yohimbine can cause respiratory depression.

There is one report of a hypersensitivity reaction including fever; chills; malaise; itchy, scaly skin; progressive renal failure; and lupus-like syndrome associated with ingestion of a one-day dose of yohimbine, a constituent of yohimbe.

Orally, yohimbine, a constituent of yohimbe, may increase urinary frequency. A case of acute renal failure has been reported for a 42-year-old male taking yohimbine. Normalization of renal function was achieved following 2 weeks of treatment with corticosteroids. The renal dysfunction was attributed to yohimbine-induced systemic lupus erythematosus.

Dosing
There is no well-established safe and standardized dose for yohimbe bark supplements. Because the yohimbine content varies so much between products, the same labeled dose can produce very different effects.
