None of beet's interactions are rated major, and the moderate ratings rest on lab research rather than effects proven in people.
The biggest group is CYP3A4 substrates, medications cleared by a particular liver enzyme; a beet pigment blocked this enzyme in lab tests, so beet could in theory raise levels of these drugs, but this has not been shown in humans. With blood pressure medications, beet's natural nitrates can relax blood vessels, though human studies are mixed and one found concentrated beetroot juice did not meaningfully change blood pressure or heart rate. People whose blood pressure medicine is closely adjusted are the ones for whom even a small added effect could matter.
Beet's long list of interacting medications reflects a few shared mechanisms applied across many drugs, not hundreds of separate proven problems. Most entries are theoretical, based on possible effects on enzymes the body uses to process medications, and the best human data so far has not shown clinically meaningful changes.

Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.
Antihypertensive Drugs
Beet and beetroot contain nitrates, which can cause vasodilation, potentially leading to lower blood pressure. However, a study published in the European Journal of Clinical Nutrition using concentrated beetroot juice found no significant impact on blood pressure or heart rate in different age groups. Other small clinical studies suggest that while beet consumption might transiently lower blood pressure due to vessel dilation, there's no consistent evidence of a lasting effect. Overall, the theoretical risk of reduced blood pressure due to beet's nitrate content exists, but studies generally indicate a low and temporary impact rather than a sustained decrease.

Overview
Beet (Beta vulgaris) is a root vegetable that has been eaten for centuries and is grown in many parts of the world. The deep-red beetroot is the most familiar form, but the leafy green tops are also edible and nutritious. Beets are related to chard and spinach.
As a supplement, beet is most often sold as beetroot juice or beetroot powder, sometimes standardized by its nitrate content. People take it hoping to support healthy blood pressure, improve exercise performance, and provide antioxidants. It is also a popular natural food coloring.
How it works
Beets are rich in dietary nitrate. In the body, nitrate can be converted into nitric oxide, a molecule that helps relax and widen blood vessels. This may improve blood flow and slightly lower blood pressure, and it may help muscles use oxygen more efficiently during exercise.
Beets also contain betalains (the pigments that give them their red or yellow color), along with vitamins, minerals, and other plant compounds that have antioxidant properties in laboratory studies. Much of the detail about how these compounds work in people is still being studied.

Possibly Effective Athletic performance
Oral beet may improve athletic performance associated with aerobic activities. However, it is not clear who is most likely to benefit, for which type of competition it is most useful, or what dose or duration of use is most appropriate.
Possibly Effective Exercise-induced muscle soreness
Drinking beetroot juice 7-8 times over 48 hours after strenuous exercise may reduce exercise-induced muscle soreness, but evidence is conflicting. <b>Details</b>: Some clinical research shows drinking beetroot juice (Love Beets Beetroot Juice, Gs Fresh Ltd.) 125 mL or 250 mL seven times over 48 hours after strenuous box jumps improves muscle soreness when compared with placebo and sodium nitrite. Other small clinical studies show that drinking beetroot juice 50-250 mL as eight separate doses over 48 hours after strenuous exerciseimproves muscle soreness and reduces tissue edema when compared with placebo.

Safety & precautions
Beet is a common food and is generally considered safe for most people when eaten or taken in normal amounts. Concentrated supplements have less long-term safety data.
People with a history of kidney stones may want to be careful, because beets contain oxalates, which can contribute to certain types of stones. People with low blood pressure should be aware that beets may lower it further.
For pregnancy and breastfeeding, beet eaten as a normal food is generally fine. However, there is not enough information on the safety of concentrated beet supplements during these times, so it is best to use caution and check with your doctor or pharmacist first.
Side effects
Beet is usually well tolerated. The most common harmless effect is beeturia — red or pink urine and sometimes red stools after eating beets. This is not dangerous, though it can be alarming if you are not expecting it.
Some people notice mild stomach upset or gas. Because beets can lower blood pressure, large amounts may cause lightheadedness in some people. People prone to kidney stones may want to limit high-oxalate foods like beets.

Serious Adverse Effects (Rare):
Orally: Hypocalcemia and kidney damage when ingested in large amounts.
Anemia Gastrointestinal
Orally, beet juice may cause red stools. This red coloring of the stools is not harmful. Additionally, beet supplementation has been reported to cause black stools. In one case, a 79-year-old male on apixaban and clopidogrel presented with black stools, nausea, and vomiting after taking beet pills 2-3 days prior. The likelihood of upper gastrointestinal bleed was determined to be low based on factors such as normal vital signs and lack of severe anemia. The patient was diagnosed with beet-induced pseudo-hematochezia which was successfully treated with fluids and discontinuation of the beet supplement.
Other less common gastrointestinal side effects include loose stools, constipation, and nausea.

Dosing
There is no official standardized dose of beet for supplement use. Studies have used a range of amounts of beetroot juice or powder, often based on nitrate content, but the best dose is not firmly established.
If you choose a beet supplement, follow the directions on the product label and do not exceed the recommended amount. Because product strength and nitrate content vary widely between brands, it is a good idea to check with your pharmacist or doctor, especially if you take blood pressure medicine.
