Based on the available evidence, the overall risk of a clinically significant Guggul drug interaction is moderate for most people, largely because guggul may speed up how the body clears many common medications. None of the interactions are rated major, but every one is rated moderate, and a few have been documented in small human studies.

Interactions deserving the most attention

Small human studies show guggul can lower blood levels of diltiazem and propranolol, which could weaken their effects on blood pressure and heart rhythm. Animal research also suggests guggul may add to thyroid hormone therapy, where even small changes can matter. Early lab and clinical findings hint that guggul could raise bleeding risk with blood thinners, though this is not well established.

What the rest of the list means

The long list mostly reflects one theory: guggul compounds may boost an enzyme the body uses to clear a large share of medications, so many drugs are flagged as a precaution. Most of these pairings come from lab research and have not been shown to cause meaningful changes in people. Other concerns, such as effects on estrogen-based medicines, also rest on lab findings alone.

Overview

Guggul is a sticky gum resin that comes from the Commiphora wightii tree (also called Commiphora mukul), a small thorny plant native to India, Pakistan, and parts of the Middle East. The resin is collected from the tree's stem and has been used for centuries in traditional Ayurvedic medicine.

In supplements, guggul is often sold as capsules, tablets, or extracts. Many products are standardized to contain a certain amount of guggulsterones, the compounds thought to be most active. People commonly take it for cholesterol, joint pain, weight goals, and skin problems like acne.

How it works

The compounds in guggul believed to be most active are called guggulsterones. Laboratory and animal studies suggest these may affect certain receptors in the body that help control how the liver handles cholesterol and bile acids.

Researchers have also looked at possible anti-inflammatory and antioxidant effects, which could explain its traditional use for joint and skin problems. However, much of this work comes from test-tube and animal studies, and it is not clear how well these effects apply to people.

Safety & precautions

Guggul is generally tolerated by many people when taken short-term, but it can cause side effects and is not right for everyone. People with thyroid problems, liver disease, or hormone-sensitive conditions should be especially careful and check with a healthcare provider first.

Because guggul may affect bleeding and hormone activity, it is often recommended to stop it before surgery. Pregnant women should avoid guggul, since it may stimulate the uterus and its safety in pregnancy is unknown. It should also be avoided while breastfeeding due to a lack of safety information. Always talk with your pharmacist or doctor before starting guggul, especially if you take other medicines or have a chronic health condition.

Side effects

The most common side effects are stomach upset, such as nausea, gas, loose stools, or belching with an unpleasant taste. Some people develop a skin rash or itching, which can sometimes be serious enough to require stopping the product.

Less commonly, headache, mild nausea, or hiccups have been reported. Allergic reactions are possible. If you notice a worsening rash, swelling, trouble breathing, or other severe symptoms, stop taking it and seek medical care.

Orally, guggul seems to be generally well tolerated.

Most Common Adverse Effects:

Orally: Belching, bloating, diarrhea, headache, nausea, unpleasant taste, and vomiting. Allergic and non-allergic skin reactions.

Topically: Allergic contact dermatitis in sensitive individuals.

Serious Adverse Effects (Rare):

Orally: Rhabdomyolysis.

Dermatitis Dermatologic

Orally, guggul can cause hypersensitivity reactions including rash and pruritus. Guggul can also cause nonallergic adverse skin reactions. The risk of skin reactions appears to be dose-dependent. In one study, the incidence of skin reactions was 3% with a dosage of 1000 mg three times daily, compared with 15% with a dosage of 2000 mg three times daily. The severity of the reactions ranged from pruritus to swelling and erythema of the face to bullous lesion on the lower legs associated with headaches, myalgias, and pruritus.

Topically, guggul can cause allergic contact dermatitis. Also, in a small clinical study, one patient using a cream containing aqueous extracts of guggul and Allium ampeloprasum as well as sesame oil complained of rash at the application site. It is unclear if this reaction was due to guggul, other ingredients, or other factors.

Dermatitis Immunologic

Orally, guggul can cause hypersensitivity reactions including rash and pruritus. In a small clinical study, two adults with hyperlipidemia developed a hypersensitivity rash, one with facial edema, within minutes of oral administration of a methanolic extract of guggul, together with Terminalia extract. It is unclear if this reaction was due to guggul, Terminalia, or other factors.

Topically, guggul can cause allergic contact dermatitis.

Muscle breakdown Musculoskeletal

There is one case of rhabdomyolysis reported in a patient who took guggul 300 mg three times daily. The patient developed hemoglobinuria within 2 weeks of starting guggul in addition to increased alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase, creatine kinase, and myoglobinemia. The patient did not have any muscular symptoms. The patient's condition improved when guggul was discontinued. The patient had a history of developing elevated creatine kinase levels after taking simvastatin; however, the patient was not taking a statin at the time of this episode of rhabdomyolysis.

Dosing

There is no single established, proven dose for guggul. Products vary widely in strength, and many are standardized to a percentage of guggulsterones. Because of this, the safest approach is to follow the directions on the product label and not exceed the recommended amount.

Before starting, check with your pharmacist or doctor to make sure guggul is appropriate for you and to confirm a reasonable dose. Do not assume that taking more will work better, since higher amounts may increase the chance of side effects.
