Based on the available evidence, the overall risk of a clinically significant Grape drug interaction appears low for most people. None of the listed interactions are rated major, and while many carry a moderate rating, nearly all rest on lab or theoretical findings rather than proven effects in people.

Interactions deserving the most attention

The interaction with the strongest human evidence involves cyclosporine, a transplant medication with a narrow margin for error; a small human study found grape juice can meaningfully reduce how much of the drug the body absorbs. Human data also suggest grape juice may lower levels of certain medications the body clears through a particular liver enzyme. In addition, grape extracts might in theory add to the effects of blood thinners and raise bleeding risk, though this comes from lab research only.

What the rest of the list means

Grape appears on many medication lists mainly because lab and animal studies suggest it could affect enzymes the body uses to process drugs. Overview

Grape comes from the plant Vitis vinifera, a woody vine grown around the world. Grapes have been cultivated for thousands of years for food, raisins, juice, and wine. They also have a long history of use in traditional medicine.

The fruit, skin, seeds, and leaves all contain natural compounds that people believe may have health benefits. Today, grape products are sold not only as food and drinks but also as dietary supplements, with grape seed extract being one of the most popular forms.

Grape supplements are often standardized to contain a certain amount of polyphenols or proanthocyanidins, which are the compounds thought to be responsible for many of the proposed effects.

How it works

Grapes contain a variety of natural compounds, including resveratrol, flavonoids, and proanthocyanidins (a type of polyphenol especially concentrated in the seeds and skin).

These compounds act as antioxidants, meaning they may help protect cells from a type of damage caused by unstable molecules called free radicals. They are also thought to have anti-inflammatory effects and may support healthy blood vessels and circulation.

It is important to know that much of what we understand about how these compounds work comes from laboratory and animal research. How well they work in the human body, and at what doses, is still being studied.

Safety & precautions

Eating grapes, raisins, and drinking grape juice is safe for most people as part of a normal diet. Grape seed extract supplements also appear to be generally well tolerated when used short term.

Who should be cautious: People with bleeding disorders, those taking blood-thinning medicines, and anyone scheduled for surgery should talk to a healthcare provider before using concentrated grape supplements, since some grape compounds may affect blood clotting.

Pregnancy: Grapes as food are fine, but concentrated supplements have not been well studied, so it is best to avoid them. Wine and other alcoholic grape products should be avoided entirely during pregnancy.

Breastfeeding: Grapes as food are fine, but there is not enough safety information on grape seed extract or other concentrated products, so caution is advised.

Side effects

Grapes and grape products are usually well tolerated. When side effects happen, they are typically mild.

Possible side effects of grape seed extract may include headache, dizziness, upset stomach, or nausea. Some people may have an allergic reaction to grapes or grape products, which can rarely be serious.

Stop using the product and seek medical care if you notice signs of an allergic reaction such as rash, itching, swelling, or trouble breathing.

Orally, the whole fruit, as well as the seed, fruit, and leaf extracts, seem to be well tolerated. Topically, grape seed extracts seem to be well tolerated.

Most Common Adverse Effects:

Orally: Abdominal pain, diarrhea, dry mouth, dyspepsia, headache, joint pain, and nausea.

Serious Adverse Effects (Rare):

Orally: Anaphylaxis to grape skin has been reported.

Orally, nasopharyngitis and oropharyngeal pain have been reported with use of a specific grape leaf extract AS195 (Antistax, Boehringer Ingelheim Pharma GmbH & Co. KG). Sore throat, cough, allergic rhinitis, and nasopharyngitis have been reported with use of grape seed extract, but these effects occur at rates similar to placebo. One case report describes a 16-year-old female who developed increased levels of immunoglobulin E (IgE) following skin-prick exposure to grape vine pollen, as well as positive test responses following bronchial and conjunctival provocation. Reduced forced vital capacity has been described in California grape workers. Occupational eosinophilic lung was diagnosed in a grape grower with a history of asthma. Respiratory exposure to sulfites in grape was implicated as the cause of the adverse reaction.

As a class, nervous system adverse effects have been reported with use of a specific grape seed extract (Entelon, Hanlim Pharm). However, the specific types of adverse neurologic effects were not described.

Constipation Gastrointestinal

Orally, abdominal pain and nausea have been reported with use of grape seed extract, but these effects typically occur at rates similar to placebo. In a case report of a 57-year-old man, intermittent nausea, vomiting, and diarrhea occurred over a 10-day period and improved once grape seed extract was stopped. Gastrointestinal adverse effects have also been reported with use of a different grape seed extract (Entelon, Hanlim Pharm). However, the specific types of gastrointestinal effects were not described. A specific grape leaf extract AS195 (Antistax, Boehringer Ingelheim Pharma GmbH & Co. KG) has reportedly caused flatulence, mild constipation, gastrointestinal discomfort, diarrhea, dyspepsia, dry mouth, and retching. Diarrhea, gastrointestinal distress, indigestion, and aversion to taste have been reported with use of Concord grape juice. Loose stools have been reported in a clinical trial of grape pomace. Bowel obstruction caused by intact grapes and grape seeds has been described in case reports. Excessive consumption of grapes, dried grapes, raisins, or sultanas might cause diarrhea due to laxative effects.
