Based on the available evidence, the overall risk of a clinically significant Dhea drug interaction appears low for most people, though a few specific medicines deserve real caution. None of the hundreds of listed interactions are rated major, and nearly all rest on theoretical or preliminary evidence rather than proven harm in people.

Interactions deserving the most attention

The clearest concern is with anti-estrogen cancer treatments such as tamoxifen, aromatase inhibitors, and fulvestrant, because DHEA acts like estrogen in the body and may work against these drugs. The best-documented interaction is with triazolam, a sleep medicine, since a human study showed DHEA raised its blood levels. There is also a theoretical bleeding concern with blood thinners, based on research showing DHEA may affect how platelets clump, and a single case report of a manic episode when DHEA was combined with an antidepressant.

Overview

DHEA (dehydroepiandrosterone) is a hormone made naturally by your adrenal glands, which sit on top of your kidneys. It is one of the most plentiful steroid hormones in the body and serves as a building block that the body can turn into other hormones, including testosterone and estrogen.

DHEA levels are highest in young adulthood and slowly drop as people get older. Because of this natural decline, DHEA has become a popular over-the-counter supplement marketed for anti-aging, energy, mood, and sexual health.

Supplement DHEA is made in a laboratory, often starting from compounds found in wild yam or soy. Importantly, your body cannot make DHEA from eating wild yam itself, so a wild yam product is not the same as a DHEA supplement.

How it works

DHEA is a precursor hormone, meaning the body can convert it into other hormones, mainly testosterone and estrogen. By raising the supply of these sex hormones, DHEA may influence mood, sex drive, bone, muscle, and tissue health.

Because hormone levels fall with age, the idea behind supplementing is to restore some of what is lost. However, more hormone is not always better, and the body's hormone systems are complex.

Much of what we understand about DHEA's effects comes from laboratory and animal studies, and the way it works in the human body is not fully understood. Effects can vary a lot between men and women and between individuals.

Safety & precautions

Because DHEA acts as a hormone, it should be treated with care. People with hormone-sensitive conditions — such as breast, ovarian, uterine, or prostate cancers — should generally avoid it, since DHEA can raise estrogen or testosterone levels.

People with liver disease, heart conditions, mood disorders, or polycystic ovary syndrome (PCOS) should be cautious and speak with a healthcare provider first. DHEA is banned by many sports organizations, so athletes should avoid it.

Pregnancy and breastfeeding: DHEA changes hormone levels and is considered unsafe during pregnancy. There is not enough information about its safety while breastfeeding, so it should be avoided then too.

Always talk to a pharmacist or doctor before starting DHEA, and remember that supplements are not as tightly regulated as prescription drugs, so quality and strength can vary between products.

Side effects

Common side effects are often related to higher hormone levels and can include oily skin, acne, and hair loss or unwanted hair growth. Some people report stomach upset, high blood pressure, or trouble sleeping.

In women, DHEA can cause more masculine effects such as a deeper voice, facial hair, or changes in menstrual periods. In men, it may cause breast tenderness or enlargement. It can also affect mood and may worsen certain psychiatric conditions.

Because it influences hormones, long-term or high-dose use raises concerns about effects on hormone-sensitive tissues. Stop use and seek medical advice if you notice troubling changes.

Orally and topically, DHEA seems to be well tolerated when used in typical doses, short-term. However, there is some concern that long-term oral use of DHEA may be linked to a greater risk for cancer.

Most Common Adverse Effects:

Orally: Acne, headache, insomnia, mood changes, and nausea. In females, masculinization symptoms including deepening of the voice, increased size of genitals, irregular menses, oily skin, reduced breast size, and unnatural hair growth. In males, aggression, breast tenderness or enlargement (gynecomastia), urinary urgency, and testicular wasting.

Serious Adverse Effects (Rare):

Orally: Possible increased risk for cardiovascular events and various types of cancer.

Acne Dermatologic

Acne has been the most commonly reported adverse effect in human research, particularly in females. However, it is generally mild and may be treated by reducing the dose. Incidences of contact dermatitis, acneiform dermatitis, greasy hair and skin, keratosis, skin rash, erythema, scalp itching, and skin spots have also been reported. Increased hair growth and hirsutism have been noted in several clinical trials, including the development of mild mustache in females. Increased perspiration and odor have also been reported in human research.

Benign prostatic hyperplasia (BPH) Genitourinary

In older adults, elevated and severe urinary symptoms (as evidenced by scores of more than 20, using the American Urological Association Symptom Index for Benign Prostatic Hyperplasia [International Prostate Symptom Score]) and urinary tract infection were reported. Rare incidences of abnormal menses and increased discharge have been reported. DHEA has been associated with hematuria.

Cough Pulmonary/Respiratory

Increased cough and nasal congestion have been noted in human research. A report of acute respiratory failure was made in clinical study evaluating the use of DHEA in patients with myotonic dystrophy (type 1).

Gastrointestinal disturbances such as nausea, diarrhea, and abdominal discomfort have been noted in human research.

Epilepsy Neurologic/CNS

In humans, dizziness, fatigue, malaise, sleep disturbances, increased dreaming, night sweats, restlessness, "painful spots," and a crawling scalp sensation have been reported. There is a case of seizure associated with DHEA use in a 30 year-old female with fragile X syndrome and no history of convulsive disorder who used DHEA to try to improve ovarian production.

Incidences of asthenia, arthralgia, and myalgia, including calf cramps, have been reported.

In postmenopausal patients, high doses of DHEA (1600 mg daily) induced insulin resistance, reportedly due to increased androgen levels that occurred during supplementation.

Dosing

There is no single standard dose of DHEA, and the right amount depends on the reason for use, the person, and the product. Doses used in research vary widely.

If you and your healthcare provider decide DHEA is appropriate, follow the directions on the product label and use the lowest amount that meets your needs. The prescription vaginal product (prasterone) has its own specific dosing set by a doctor.

Because DHEA is a hormone, it is wise to have a pharmacist or doctor guide your dose rather than self-prescribing. Do not assume a higher dose is safer or more effective.
