Overview

Devil's claw (Harpagophytum procumbens) is a plant native to the dry regions of southern Africa, including the Kalahari Desert and parts of Namibia and South Africa. It belongs to the sesame family (Pedaliaceae). The plant gets its name from the small hooked, claw-like fruit it produces.

The part used in supplements is the dried secondary root, sometimes called the tuber. People have used devil's claw in traditional African medicine for many years for pain, fever, and digestive complaints.

Today it is most often taken as a capsule, tablet, tea, or liquid extract for joint and back pain. Many products are standardized to a compound called harpagoside.

How it works

Devil's claw contains compounds called iridoid glycosides, the best known being harpagoside. These are thought to be the main active ingredients.

Laboratory and animal research suggests these compounds may lower the body's production of certain inflammatory chemicals, which could reduce pain and swelling. This is why devil's claw is often marketed as a natural anti-inflammatory.

It is important to know that much of this understanding comes from test-tube and animal studies. Exactly how it works in the human body, and how much of the active compound reaches the joints, is still not fully understood.

Because it may increase stomach acid, people with stomach or duodenal ulcers or other digestive problems should use caution. It may also affect blood sugar, blood pressure, and heart rhythm, so people with diabetes, heart conditions, or gallstones should check with a healthcare provider first.

It is best to avoid devil's claw during pregnancy, since it may affect the uterus, and to avoid it while breastfeeding because there is not enough safety information. Stop taking it before any planned surgery and let your care team know you have been using it.

Side effects

The most common side effect is digestive upset, such as diarrhea, nausea, stomach pain, or heartburn.

Less commonly, people report headache, dizziness, ringing in the ears, or changes in appetite. Allergic reactions are possible but uncommon.

Anorexia Neurologic/CNS

In a trial of devil's claw, one patient withdrew after 4 days of therapy due to a throbbing frontal headache, as well as tinnitus, anorexia, and loss of taste. Rarely, dizziness, somnolence, and insomnia have been reported. It is unclear if these symptoms were caused by devil's claw.

Anorexia Gastrointestinal

Gastrointestinal side effects, including mild gastrointestinal upset, diarrhea, anorexia, acid reflux, or loss of taste, have been reported in some individuals receiving devil's claw, especially at high doses. Gastrointestinal complaints have been reported in 9% to 18% of patients taking a specific devil's claw extract (Doloteffin, Ardeypharm), while diarrhea was reported in about 8% of patients taking devil's claw (Harpadol, Arkopharma). Several cases of gastrointestinal bleeding have been reported.

Anxiety Psychiatric

Rarely, anxiety has been reported in patients taking devil's claw.

Dysmenorrhea Genitourinary

Dysmenorrhea was reported in one patient taking a specific devil's claw extract (Doloteffin, Ardeypharm) for 8 weeks.

Hypertension Cardiovascular

In one case report, a healthy patient with normal blood pressure presented with hypertension after taking devil's claw 250 mg twice daily for 2 weeks. It gradually resolved after discontinuation of devil's claw. Some animal research shows that devil's claw might have negative chronotropic, as well as positive and negative inotropic, effects. However, these effects have not been documented in humans.

Nausea and vomiting Endocrine

In one case report, a 65-year-old female developed psychomotor agitation, nausea, and distress from euvolemic hyponatremia secondary to inappropriate secretion of antidiuretic hormone (SIADH) within 1 month of starting daily treatment with devil's claw. Within 5 days of discontinuing the product and receiving sodium replacement, the symptoms resolved. Two months later, the patient re-initiated devil's claw and again developed euvolemic hyponatremia.

Dosing

There is no single official dose for devil's claw. Many products are standardized to their harpagoside content, but the amount varies a lot from one brand to another.

The safest approach is to follow the directions on the product label and not take more than recommended. Capsules, tablets, teas, and liquid extracts are all used, and the right amount depends on the specific product.

Because strength and quality differ between brands, it is a good idea to check with your pharmacist before starting, and to choose products from reputable companies that test their supplements.

When used orally. Anecdotal evidence suggests that devil's claw has oxytocic effects in humans. Also, in vitro research shows that moderate to high doses of devil's claw root extract induce contractions of isolated uterine muscle from pregnant and nonpregnant rats; avoid using.
